Central sympatholytics

Methyldopa, clonidine, and guanabenz are thought to act on brainstem adrenoreceptors, thereby stimulating centers in the brainstem that inhibit sympathetic outflow.

Potential adverse effects include orthostatic hypotension, dry mouth, sedation, and impotence. In addition, clonidine may be associated with constipation and a severe rebound hypertensive response following sudden cessation.

Reserpine

Reserpine lowers blood pressure by depleting peripheral catecholamine stores. Sedation is the most common side effect; depression can occur at higher doses. The drug may still be useful for some patients and has the advantages of low cost and once-a-day dosing. An increased incidence of breast cancer, previously reported in patients receiving this drug, has not been confirmed.
